To improve your experience we offer all of our forms online so that our new patients can have them filled out when they arrive for their exam. By doing so you will get back to see the doctor much quicker.
What should I bring to my exam?
- Any current eyewear such as glasses, computer glasses, sunglasses, etc.
- If you wear contact lenses please bring either your boxes or foil tops so we know exactly what brand and powers you are currently wearing
- ALL insurance cards (medical and vision).

Dr. Troy White Eyecare is an out of network provider for most vision insurance plans.
When you make an appointment please make sure you give us both your current vision and medical insurance.
Why is this important?
We want everyone one of our patients to have a great experience when you visit our office. If we have all of your insurance information ahead of time we can check all your benefits and have them ready when you come in for your appointment. If a problem arises, it gives our office time to contact you ahead of your appointment to inform you and get this issue resolved before your appointment. By getting all this information in advance it often minimizes wait time because we verify all benefits before you see the doctor.
Often times these plans can be confusing to understand because every plan is different and each plan may provide discounts for certain services or materials. Often insurance may only provide discounts toward the eye exam. Sometimes they provide discounts that can be used to reduce the cost of frames and/or lenses. Some insurance plans even provide discounts that can be used to reduce the cost of your contact lens fitting and/or purchase of contacts. We know many patients do not understand all of this so we do our best to explain your benefits during your visit. Please feel free to ask any questions if there is something about your insurance plan that is unclear and we will do our best to help you understand.
